Care Coordinator

Carina’s role at CURIS as a Remote Care Coordinator is to support our community health center clients by managing patient care outside of in-person visits. This role involves coordinating follow-up care, conducting outreach, assisting with referrals, and supporting chronic disease management—often through phone or digital platforms. By working closely with clinical teams, Remote Care Coordinators help improve access, enhance care continuity, and support better health outcomes for patients, especially those in underserved or rural areas.

Carina Perea is a bilingual Certified Medical Assistant with experience in clinical care, telehealth coordination, and marketing. She holds a Bachelor’s degree in Biology and has worked across various healthcare settings, supporting patient care, administrative functions, and digital outreach. Carina combines strong communication skills with hands-on medical knowledge and a creative background in marketing and social media.
