Clinical Nurse Care Manager

As Clinical Nurse Care Manager at CURIS, Evelyn plays a key role in advancing quality improvement, quality assurance, risk management, and operational efficiency. She helps lead CURIS’ clinical services while supporting the integration of AI technologies to streamline workflows and enhance care delivery. Evelyn is deeply committed to helping health centers improve patient outcomes, strengthen workforce retention, and optimize clinical operations wherever support is needed.

Evelyn is a dedicated healthcare professional with over a decade of experience in clinical operations, quality improvement, and patient-centered care. Previously serving as the Quality Improvement Leader at Lifecare Family Health and Dental Center, she oversaw risk management, QA/QI programs, UDS compliance, FTCA coordination, and PCMH certification, while also leading care coordination and patient engagement initiatives.

With a strong clinical background as an LPN, Evelyn has held various frontline and leadership roles, including COVID-19 clinic coordination, diabetic education support, and EMR training. Her career spans hospital, academic, and community health settings, and she brings hands-on experience in both clinical care and administrative functions. She is passionate about using data to drive decision-making and improve patient outcomes.

Evelyn holds an LPN-M-IV certification from Brown Mackie College and was honored with the Community Health Center Hero Award in 2018 for her commitment to excellence in community health.
