Registration, Guidelines, and Payments
Attendance Fee Structure:
Individual Registration: $1,175
Group Registration: Groups of 10 or more receive a 10% discount.
*Travel and hotel accommodations are not included in the registration fee and are the responsibility of each attendee. Please refer to the event agenda for details on meals included with registration.
Please use the form below to register yourself and/or your group and reserve your spot for Health Center Forward. Once we receive your completed registration form, a member of the CURIS team will follow up with a link to submit payment.
Please note: Registration is not confirmed until both the registration form and payment have been received.
Cancellations and refund requests will be accepted through September 30, 2026. After this date, registration fees are non-refundable; however, the amount paid may be applied as a credit toward a future CURIS service.
